Syringocystadenoma papilliferum (SCAP) is a rare, benign, hamartomatous, adnexal proliferation arising from apocrine glands, most commonly occurring in the scalp. Its presentation in the leg has been occasionally reported in literature, in association with an organoid nevus or apocrine nevus. Here, we present the case of a 29-year-old primigravida with a pigmented, keratotic papule on the medial aspect of the right leg with a clinical suspicion of perforating disorder. At histopathology, it was diagnosed as SCAP associated with an apocrine nevus. This case is being reported due to the unusual location.
