The purposeof this letter is to raise awareness among researchers andclinicians and to stimulate further investigation of this issue. The rising phenomenon of chemsex— the intentional useof psychoactive substances such as methamphetamine,3,4-Methylenedioxymethamphetamine (MDMA), and gammahydroxybutyrate (GHB) to enhance sexual experiences—posesa hidden mental health crisis in India, particularly among men,who have sex with men (MSM) and transgender communities.Despite its prevalence in urban centers such as Delhi, chemsexremains under-researched in India, leaving vulnerablepopulations at risk for severe psychiatric consequences. A recent Indian online survey highlights the perceived harmsand patterns of chemsex among MSM, emphasizing the needfor systematic exploration in this area.

Detailed reporting of such cases is criticalfor understanding the psychiatric sequelae of chemsex, refining targeted interventions, and informing harmreduction strategies. By contributing to this growing bodyof knowledge, clinicians can address the unique challengesposed by this emerging public health concern.
